The following are necessary to enroll at FHS:
1. Birth Certificate - State or County version, NOT from the hospital (hospital certificates are not legal documents). Your student must be registered using his or her legal name.
2. Transcript of grades/credits from previous school.
3. Immunization Record to include:
a. 3 Polio (IPV) (1 dose must be given after the 4th birthday)
b. 4 Diphtheria/Tetanus/Pertussis (DTP/DT/DTaP/Td) (1 dose must be given after 4th birthday)
c. 2 Measles/Mumps/Rubella (MMR) (1 dose given on/after 1st birthday and 1 dose given prior to Kindergarten)
4. Verification of Address - Current (within 30 days) utility service bill (electric, gas, internet, land line phone) or work order for utility service or mortgage/rental receipt or purchase agreement. Document must include your name and the address to which direct service is provided, sufficient to prove you are residing at the location. No other documents (mail, driver's license, etc.) will be accepted for address verification.
5. Fully Completed Enrollment Forms - Forms available for pickup from the registrar
6. Other Documents may be required if special circumstances apply, such as request for enrollment of a non-resident student, request for in-district transfer, no owned/rented living accommodations, parent to parent transfer.
Registration cannot proceed without above documents or without the presence of a legal parent/guardian AND the student.
If an appointment is made to register, and the student arrives without a legal parent or guardian, student will have to re-schedule their registration meeting. Guardianship papers must be notarized, original (no copies) and be included with documents listed above. Identification can be required at any time of parent or guardian.
